To provide an update and probable closure on this question...

I unearthed my spare TDCi manifold over the weekend. All the fixing holes for the head fixings are 12.9mm diameter with the exception of the top hole on the number 3 port, which is 10.3mm (as are the EGR passage fixing holes), The studs and bolts are all M10, so a very sloppy fit in all but one hole.

The spacer fits within the manifold hole and reduces the diameter of the hole into which it is fitted from 12.9mm to 10.3mm, so the only conclusion is that it is simply to provide alignment for the manifold on the head. It probably isn't critical, and is probably best fitted to the top hole on number one port since this is the greatest distance of any from the other locating hole.

Ok, so it provides angular alignment as both X and Y alignment is already determined by the smaller hole in the manifold.

That is a very strange way of getting alignment. Probably an elongated hole (12.9x10.3) on the #1 top hole would have done the job better from an engineering perspective, and removed one part from the spares bin. But maybe Ford found that it would be cheaper to drill than mill, and create a plastic insert. 1988 90 Hard Top, 19J Diesel Turbo, Shire Blue - Restoration ongoing

I think LR found another way to remove the part from the bin, by simply not fitting it. It wasn't there on my 2007, and it wasn't there on my spare engine (ex-2009 write-off). Both engines, as far as I know, had never had the manifold removed before.

The other possibility is that the plastic spacer simply burns away over time! Rolling with laughter
