Try Shell fuel or Esso or the like permanently and no supermarket fuel. IF you still get it happen at some point you can then rule that out as being an issue at the very least.

lack of it! sounds like it can't keep up with the supply a higher engine speeds. i guess its ok around town etc?

Air flow meters can be a problem on the D4D engines, check for fault codes with an obd reader, those codes on jap systems rarely bring on the management light. I plugged in the reader on my wifes rav and there was over a page of faults not ever bringing the light on erased them all and the car now flies. If everything is under control you are not going fast enough.

sounds identical to the problem I had with a Mitsubishi Outlander. It turned out to be a gunged up MAP sensor. This happened twice within a year and after 2 replacements I cured the problem by using 2 stroke oil in the diesel which must have kept everything clean
