I got fed up with the same issue and ended up replacing the spline adapters with the later type that has the retaining nut. No more issues.

Had a leather type seal in mine, local specialist recommended replacing with a newer rubber type seal, so they fitted the newer type flange and rubber seal, been happy with it so far.
