Is it a 2015 model with DSC?

Mine is 2014 I don't believe this year had DSC.

Has the vehicle got any mods / lift / suspension changes?

Only mods have been on for about 4 years, wheel spacers and slightly larger tyres 285's. thats it, rest is standard.
